Submit A Ballot
A Ballot Deadline of December 2, 2019 has been set in this matter for the submission of ballots. All ballots must be actually received by Omni Agent Solutions no later than 05:00 PM - EST by this date.
Balloting Submission Instructions
You may file your ballot via either of the following methods:
Hardcopy submission (by mail, overnight, courier, or hand delivery)
Registering for e-Ballot Submission
To file your ballot online, you must register to access the online submission account page. Click here to register for online ballot submission. You will need a working email address to register for online ballot submission. Once you register, you will receive an email message that contains a link to activate your registration. Once activated, you may login and submit your ballot as described below.

Once you have registered for e-Ballot Submission and have activated your online e-Ballot submission account, you may log in to your account by clicking here.
Hardcopy Submission (by mail, overnight, courier, or hand delivery)
All signed, original Ballot forms, along with accompanying documentation, must be filed at the following address:

Ballots may be submitted in person, by courier service, overnight delivery service, hand delivery or mailed to the Balloting Agent at the foregoing address.
