You must submit an originally executed proof of claim.

If you would like a copy of your claim returned to you as proof of receipt, please enclose an additional copy and a self-addressed postage-paid envelope.

Privacy of Information Collected

Your proof of claim form must not contain medical records, complete social security numbers or tax identification numbers (only provide the last four digits), a complete birth date (only provide the year), the name of a minor (only provide the minor's initials) or a financial account number (only provide the last four digits of such financial account).

PLEASE REVIEW YOUR PROOF OF CLAIM AND SUPPORTING DOCUMENTS AND REDACT ACCORDINGLY PRIOR TO UPLOADING THEM. PROOFS OF CLAIM AND ATTACHMENTS ARE PUBLIC DOCUMENTS THAT WILL BE AVAILABLE FOR ANYONE TO VIEW ONLINE.

The information requested on the proof of claim form is being collected for the purposes of facilitating the debtors’ bankruptcy cases under the U.S. Bankruptcy Code and processing any claim you may have against the debtors. Some or all of the information you provide on the proof of claim form could be displayed on this case website and may be shared with certain third parties in furtherance of the bankruptcy case and process. Applicable law or court order may prohibit the amendment or erasure of such information once it is submitted.

With respect to General Proofs of Claim submitted on behalf of minors and by individuals with claims arising from sexual abuse who were at least eighteen (18) years old at the time the sexual abuse began:

In this case, the Bankruptcy Court has set up a procedure to protect your privacy. In order to protect your privacy, you must file according to the directions above. Do not file your General Proof of Claim with the Bankruptcy Court. General Proofs of Claim submitted on behalf of minors will not be available to the public. General Proofs of Claim submitted by individuals with claims arising from sexual abuse who were at least eighteen (18) years of age at the time the sexual abuse began will not be available to the general public unless such claimant requests, solely in his or her discretion, for such proof of claim to be made public by contacting the Claims and Noticing Agent. These Proofs of Claim will only be confidentially provided to the Permitted Parties as defined in the Bar Date Order pursuant to Bankruptcy Court-approved guidelines.
