You will need a working email address to register for online claims submission. Once you register, you will receive an email message that contains a link to activate your registration. Once activated, you may login and submit your claim as described below.

Submitting a Claim Online

Once you have registered for online claim submission and have activated your online claims submission account, you may log in to your account by clicking here. You may submit multiple claims in a single log-in session, and you may log in at any time during the duration of the case to submit additional and/or amended claims.

Hardcopy Submission (by mail or hand delivery)

All signed, original Proof of Claim forms, along with accompanying documentation, must be filed at the following address:
